Building it right from day one
A qualification file assembled correctly at hire — application, MVR, medical certificate, road test certification, and the previous-employer safety performance history — is far less work to maintain than one pieced together retroactively during an audit response, when documents may be harder to obtain and gaps are harder to explain.
The annual review that's easy to miss
391.25 requires an annual review of each driver's MVR and a note in the file confirming the driver still meets minimum qualification standards, and 391.51 requires the full file be retained for the length of employment plus three years after — requirements that are straightforward on paper but easy to let slip without a system tracking the due date across every driver in the fleet.
